I agree with this. The system is the problem and the system needs to be put to account. The rot starts from the top down and it's a pyramid of 'lack of oversight' being drilled down to 'operators doing whatever they want'. If the top was fixed the bottom wouldn't be possible.

Heaps of outcomes have been put into place as a result of that report. People were stood down, new commands put in place. Systematic training changes instated. Thing is, very little of that has been bleated by the MSM because punishments aren't sexy. Trials are. Intrigue is. The story broke and then the public at large no longer cared. And as a result anyone that is questioning why there isn't anything in the news about it simply don't realise that the general public didn't care past the last outrage news cycle so no one bothered reporting it. It is what it is.
